Forms

Authorization for Medication: Gives TCCC permission to administer medication to your child. This form must be signed both by a parent, and also filled out and signed by your child's physician.

Parents' Night Out Information Sheet: Provides information required for children to participate in Parents' Night Out.

Permission To Apply Sunblock, Lip Balm, & Diaper Cream: Gives authorization for staff to apply sunscreen, lip balm, and diaper cream to your child. No doctor's permission as necessary.

Helping Hands Time/Expenses Slip: Form to get credit for your Helping Hands time and expenses.

Corporate Sponsor Agreement: Form to apply for Corporate Sponsorship and to enroll employees in the annual Priority Enrollment/Tuition Discount Sponsor Plan.

Birthday Order Form: For a birthday cake or cupcakes to be delivered to your child's classroom on his/her birthday, complete and submit this form to the Director of your Center about a week before the day you want it delivered.

Field Trip Permission Form: Form facilitates specific communication with TCCC regarding your child’s participation/non-participation in an indicated off-site activity.

Emergency Card Form: This card contains critical information about your child that must be on file in the event of an emergency situation. If this information changes, you must ensure that you update the emergency card immediately. This request is mandated by VA State Licensing regulations.

Enrollment Form: This form is not available online. Please contact the Center Directors for a tour of each location.

School's Out: Weather closings or emergency notices via e-mail or text message

Tysons Corner Children's Center is a member of the e-mail alert system: SchoolsOut.com. If there is an emergency (e.g., heavy snowfall, power outage, etc.) at TCCC that requires the hours of operation to change SchoolsOut.com will display the current operation status of the center as updated by the Executive Director or other designee. In addition to displaying the information, the site will distribute up-to-date information via e-mail to its subscribers.

We encourage you to register to receive e-mail notification of center alerts by going to www.SchoolsOut.com and clicking on Child Care & Preschool/Virginia/Tysons Corner Children's Center/I WANT E-MAIL, fill in your e-mail address and click on the subscribe button. E-mail addresses are kept confidential.

TCCC List-Serv: Important information regarding center activities sent directly to your e-mail

The TCCC list-serv is an e-mail update program designed to deliver important TCCC-related information to parents. Most information regarding Center business will be distributed primarily through the list-serv, however, there will be times when you will also receive information via your child's cubbie or clipboard. To make sure you don't miss critical information regarding the Center and your child(ren), please make sure you are signed up.

There are a number of ways to join, but the easiest way is to send a blank e-mail to:

You will receive a confirmation message. Just reply to this message and your subscription will be complete. E-mail addresses are kept confidential.

FAQs

Will I be able to post messages to the list? No. This is an announcement-only listserv that will be used by the TCCC Executive Director or other designee.

Will I be able to respond to messages through the list? No. Since this is an announcement-only listserv, only owners and moderators can post messages - all replies are sent to an unattended mailbox and deleted. They will not be read or responded to.
